A systematic evaluation of the patient should identify those conditions requiring immediate surgery. Fortunately these are not as common as the less dramatic conditions which should respond to conservative treatment. However, regular monitoring of the patient's condition is essential and the clinician must always be prepared to adopt a more aggressive approach.
